How to use this document: These Terms apply to all visitors and clients. Sections labeled Medical Track apply to clients receiving telemedicine services in current medical-service states (CA, FL, IL, MA, MI, MS, NC, NH, NY, TX, UT, WA). Sections labeled Coaching Track apply to clients receiving health coaching services. Sections labeled All Clients apply universally.

Part I: Website Terms of Use

1.1 Acceptance of Terms

By accessing or using hillarylinmd.com (the "Website"), operated by Hillary Lin, MD ("Dr. Lin," "we," "us," or "our"), you agree to be bound by these Terms of Service and all applicable laws and regulations. If you do not agree to these Terms, please do not use the Website.

1.2 Permitted Use

You may use the Website for lawful purposes only. You agree not to:

  • Use the Website in any way that could damage, disable, or impair the Website or interfere with any other party's use
  • Attempt to gain unauthorized access to any portion or feature of the Website or any connected system
  • Harvest or collect email addresses or other contact information from the Website
  • Use the Website to transmit any unsolicited or unauthorized advertising, promotional materials, or spam

1.3 Intellectual Property

All content on this Website, including articles, podcast episodes, images, graphics, course materials, newsletters, and other materials, is owned by Hillary Lin, MD or our licensors and is protected by copyright and other intellectual property laws. You may not reproduce, distribute, modify, or create derivative works without our express written permission. Personal, non-commercial use with proper attribution is permitted.

1.4 Educational Content Disclaimer

Content published on this Website (including articles, newsletter archives, podcast episodes, and social media) is for informational and educational purposes only. It does not constitute medical advice, diagnosis, or treatment. Viewing Website content does not create a physician-client relationship. Always consult a qualified healthcare provider for medical decisions.

1.5 Newsletter

By subscribing to The Longevity Letter or any other newsletter, you consent to receive email communications from us. You may unsubscribe at any time by clicking the unsubscribe link in any email we send. We do not sell subscriber lists.

1.6 Third-Party Links

The Website may contain links to third-party websites. We are not responsible for the content, privacy practices, accuracy, or availability of such websites. Your use of any third-party website is at your own risk and subject to that site's terms.

1.7 Public Care Inquiries

The public care inquiry collects limited operational information: name, email, location, a structured description of care expectations, and acknowledgments of the practice's access and local-care boundaries. Do not submit symptoms, diagnoses, medications, records, or other medical details through this form. The practice reviews location, scope, expectations, and capacity before deciding whether to offer a private Foundation purchase link.

If the inquiry is accepted, the practice sends a private link to review and purchase Foundation. Living Care is not available for direct public purchase. The inquiry does not create a physician-patient relationship or guarantee acceptance, an appointment, appointment timing, Foundation capacity, or ongoing Living Care. Medical history is provided later through protected clinical intake. Foundation is a bounded first clinical cycle. The practice decides separately, with the patient, whether continuing Living Care is mutually appropriate and whether capacity is available.

Medical Track

Part II: Telemedicine Medical Services

This Part II applies to clients receiving telemedicine medical services from Hillary Lin, MD. Medical services are available only to individuals who are physically located in a state where Dr. Lin holds a valid, active medical license at the time of each service visit.

Current medical-service states: California, Florida, Illinois, Massachusetts, Michigan, Mississippi, North Carolina, New Hampshire, New York, Texas, Utah, Washington. Final eligibility and licensure availability are confirmed at the time of service. NPI: 1023403904.

2.1 Scope of Medical Services

Telemedicine services may include clinical consultation, assessment, diagnosis, prescription management, laboratory ordering, and follow-up care, as clinically appropriate and as determined by Dr. Lin's professional judgment. All telemedicine services are held to the same standard of care as equivalent in-person medical services.

2.2 Client Location and Eligibility

Paid care offerings are available only to adults age 18 or older. By purchasing, you confirm that you meet this age requirement.

You represent and warrant that you are physically located in a licensed state at the time of each service. You agree to confirm your physical location at the start of each visit. Services provided to individuals outside current medical-service states are strictly prohibited. Misrepresentation of your physical location may result in immediate termination of services and may constitute a violation of law.

2.3 Foundation and Continuation of Care

Foundation is a one-time $1,350 purchase for one bounded first clinical cycle. It includes records review and the initial physician evaluation; the first Living Care Map and initial plan; initial laboratory or imaging orders and prescriptions when clinically appropriate; and one bounded closure review of testing Dr. Lin orders as part of that initial plan, provided in writing or through a brief follow-up according to clinical complexity. The date the initial physician evaluation is completed is the "Initial Evaluation Date." That date anchors all later Living Care billing dates if Living Care later begins. Foundation's included service period ends exactly three calendar months after the Initial Evaluation Date whether or not Living Care begins. That boundary limits the included Foundation services; it does not erase the practice's separate duties to reconcile orders, results, safety communications, or transfer obligations it initiated.

Foundation payment or completion does not renew, does not create recurring-payment consent, does not guarantee Living Care, and does not guarantee capacity. Continuing Living Care begins only after the practice offers it privately and the patient takes a separate affirmative enrollment action. If Living Care begins, the Foundation evaluation is the first expected physician visit and Foundation covers the first three-month service term.

Continuing Living Care is offered privately only when Dr. Lin and the patient agree that it is appropriate and practice capacity allows. Quarterly and annual billing include the same clinical care. The expected baseline is about one physician visit around each quarter, with the Foundation evaluation serving as the first. This is a planning rhythm for a bundled physician relationship, not a bank of visits, and Living Care is not unlimited or on demand. No routine per-visit fee applies. A clinically appropriate out-of-cadence visit may be scheduled without an additional HLM visit fee when appropriate for the plan and when capacity allows. Labs, imaging, medications, external clinicians and facilities, and other third-party costs are separate.

The practice tracks orders and results it initiates through receipt, clinical review, patient communication, and appropriate escalation or documented closure or transfer, whether or not Living Care begins. Foundation includes the bounded first-cycle work described above; it is not a promise of indefinite care. Additional testing rounds, delayed reassessment, treatment changes, ongoing titration, serial monitoring, repeated refills, new medication work, or continuing authorization work require continuing Living Care if offered or a transfer to a local clinician who accepts management.

Living Care is not concierge primary care, on-call care, urgent care, or unlimited access. Patients must keep a local pathway for emergency, urgent, physical-exam, and in-person needs. Brief, nonurgent questions may be sent to the practice inbox, which is reviewed on business days with a response within two business days. New symptoms, multiple questions, substantial result interpretation, or medical decisions may require a visit. Messaging is not continuously monitored and must not be used for urgent or time-sensitive care.

2.4 Telemedicine Informed Consent

Before receiving telemedicine services, you are required to review and agree to our Telemedicine Informed Consent. That document is incorporated by reference into these Terms and governs the clinical aspects of your care, including technology limitations, privacy, and your rights as a client receiving care via telehealth.

2.5 HIPAA and Privacy

Medical information from medical services is handled under the medical practice's privacy practices and the Notice of Privacy Practices provided during patient onboarding, and under applicable health-privacy law, including HIPAA where applicable. The website Privacy Policy describes general website, newsletter, coaching, and program data practices; it is the website and program privacy policy, not the medical practice's Notice of Privacy Practices. The practice uses safeguards appropriate to the information and services.

2.6 Prescribing Policy

Dr. Lin may prescribe medications as clinically appropriate. Prescriptions are subject to applicable state and federal law, DEA regulations, and Dr. Lin's clinical judgment. Certain medications may require an in-person visit or additional review. Dr. Lin reserves the right to decline any prescription request. Prescriptions for weight management, hormone therapy, peptides, and other specialty categories are subject to additional clinical evaluation requirements.

2.7 Emergency Services Disclaimer

Telemedicine is not suitable for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Dr. Lin does not provide emergency medical services and telemedicine is not a substitute for emergency care.

2.8 Limitation of Liability (Medical Track)

To the maximum extent permitted by applicable law, Dr. Hillary Lin, MD shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of telemedicine services. Our total liability for any claim arising from telemedicine services shall not exceed the amount you paid for services in the three months preceding the claim.

This limitation does not apply to: (1) gross negligence or intentional misconduct; or (2) liability that cannot be limited under applicable law, including applicable state medical liability statutes.

Important notice: Medical malpractice liability is subject to state-specific statutes and caps. The limitations above are intended to apply only to the extent permitted under the law of the state in which services were rendered. Nothing here limits any rights you have under state medical liability law.

Coaching Track

Part III: Health Coaching Services

This Part III applies to clients receiving health coaching services. Health coaching is available to clients in any U.S. state. Clients outside current medical-service states may receive coaching services only, not medical services.

3.1 Nature of the Coaching Relationship

Health coaching services constitute an educational and supportive relationship, not a medical, clinical, or therapeutic relationship. Hillary Lin, MD is a licensed physician in certain states. However, health coaching services do not constitute the practice of medicine and do not create a physician-patient relationship.

By engaging in health coaching services, you acknowledge and agree that:

  • No physician-patient relationship is established
  • No medical advice, diagnosis, treatment, prescription, or referral is being provided
  • Coaching content is educational and informational, not clinical
  • You are solely responsible for decisions you make about your health

3.2 Longevity Strategy Coaching Tiers

Longevity Strategy Coaching is an education, structure, and accountability service. Core provides the productized program: intake, a baseline report, a dashboard and tracker, weekly adaptive reports, education modules, and prompts to help you ask better questions of your own clinician. Core does not include one-on-one access, replies to messages, or calls.

Guided adds a structured weekly check-in with one bounded response from Hillary each week. Private adds deeper, higher-context strategy work. Internal review of program outputs for quality and safety may happen at any tier and is not personal medical review or a promise of individual attention beyond the tier you purchased.

If you upload labs, scans, records, or biomarker data, we organize it as source material and may reference it in educational content and clinician-question prompts. This is metrics literacy, not medical interpretation. Decisions about testing, medications, and treatment belong with you and your licensed clinician.

Medical services are separate. In limited cases, and only where Hillary Lin, MD is licensed and able to serve you, you may be separately offered medical services through distinct medical-practice workflows, with their own consent, records, and terms. Coaching and medical care are separate services, and nothing in coaching converts into medical care automatically.

3.3 Health Coaching Agreement

Before receiving coaching services, you are required to review and agree to our Health Coaching Agreement. That document is incorporated by reference into these Terms and governs the scope, limitations, and disclaimers applicable to coaching services.

3.4 State-Specific Coaching Disclosures

Note: Coaching services may be available in any U.S. state, including states where Dr. Lin also offers medical services, when the purchased service is clearly designated as coaching. If you are seeking diagnosis, treatment, prescriptions, lab ordering, or other medical services, you must use the Medical Track and be physically located in a state where Dr. Lin is authorized and able to provide care at the time of service. Coaching does not become medical care merely because you are located in a medical-service state.

3.5 No HIPAA Obligation (Coaching Track)

Health coaching services are not covered under the Health Insurance Portability and Accountability Act (HIPAA). Your coaching information is protected by our Privacy Policy but is not subject to HIPAA-mandated protections applicable to medical records. By engaging in coaching services, you acknowledge this distinction.

3.6 Your Responsibility for Medical Care

You agree that you are responsible for maintaining your own medical care with a licensed healthcare provider. Coaching does not replace medical care. You represent that you have access to a physician or healthcare provider for your medical needs.

3.7 No Guarantee of Results

Health outcomes vary by individual. No particular outcome, result, weight loss, cure, or health improvement is guaranteed by participation in health coaching. Testimonials from prior clients represent individual experiences and may not be typical.

3.8 Limitation of Liability (Coaching Track)

To the maximum extent permitted by applicable law, Hillary Lin, MD and any related coaching entity shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your participation in coaching services or your use of any coaching information provided.

Our liability is limited to the fees paid for the specific coaching session or program at issue. Health coaching services are not medical services and are not subject to medical malpractice standards.

All Clients

Part IV: General Provisions

4.1 Cancellation and Refund Policy

Consultation appointments: Appointments cancelled with less than 24 hours notice may be charged a cancellation fee at Dr. Lin's discretion. Emergency cancellations are considered individually.

Rendered services: Fees for services that have been provided are generally non-refundable. If a technical failure on our end prevents a scheduled telehealth visit from occurring, a full refund or complimentary reschedule will be offered.

Foundation and Living Care billing: Foundation and continuing Living Care are offered only through private, practice-authorized paths. Living Care is not available for direct public purchase. Foundation payment, a completed evaluation, or a payment method on file does not authorize a recurring charge. Continuing Living Care requires a separate, affirmative selection under the then-current authorization and legal terms.

Foundation is a one-time $1,350 purchase for the bounded first clinical cycle described in Section 2.3 and does not renew. The Initial Evaluation Date anchors every later billing date if Living Care begins. Payment or completion does not guarantee that Living Care will be offered or that capacity will be available.

Quarterly billing: The Living Care charge is $0 at continuation selection because Foundation covers the first three-month service term. The first recurring charge is $1,350 exactly three calendar months after the Initial Evaluation Date. A $1,350 charge then occurs every three calendar months until canceled, with no annual commitment. Cancellation stops future quarterly charges; the current paid term continues through its end.

Annual billing: The first-year balance is exactly $3,750 after a verified $1,350 Foundation credit. The first full $5,100 annual renewal occurs one calendar year after the Initial Evaluation Date and renews for exactly $5,100 annually until canceled. Annual billing provides a $300 savings compared with four quarterly terms and does not change the clinical care included.

Initial annual step-down: A valid step-down request must be received before the exact deadline 30 days after annual enrollment; a request received at the deadline is not timely. When the request is valid, the practice stops annual renewal and refunds exactly $3,750. The Foundation-covered first quarterly term remains, and the next $1,350 charge is exactly three calendar months after the Initial Evaluation Date, with quarterly billing continuing every three calendar months until canceled. At or after the deadline, ordinary cancellation only stops the next annual renewal, and the current paid annual term continues.

Unintended, unused annual renewal correction: If a $5,100 annual renewal was unintended and no services were provided or used in the renewed term, a correction request must be received before the exact deadline seven days after the authoritative renewal charge. The practice verifies no use through the refund; a request alone does not guarantee a refund. If verified, the practice stops the next renewal and refunds exactly $5,100. If service use is detected or the required evidence is unavailable, no unused-renewal correction is available. At or after the deadline, ordinary cancellation only stops the next annual renewal; the paid term continues.

Use Manage billing to cancel or manage Living Care. If you cannot use the billing page, please contact support. Paid-term fees are not prorated or refunded except where required by law, under the exact step-down or unused-renewal correction above, or when the practice cannot provide purchased services.

Billing cancellation, a refund, nonrenewal, or the end of a financial term does not by itself close clinical care. Financial status and documented clinical closure or transfer are separate. If clinical care ends, the practice communicates that separately and handles active orders, results, medication monitoring, safety communications, and transition duties as applicable. This separation does not promise care indefinitely.

People enrolled under legacy Core or Premium plans retain their accepted terms and pricing through the end of their current paid term. A valid, unexpired, version-bound legacy checkout authorization may be completed under the contract affirmatively accepted for that authorization. Those legacy plans are not offered to new patients and are not silently rewritten. Migration to current Living Care occurs only by affirmative member choice and current authorization; no new legacy offer is available.

Superbills for potential insurance reimbursement are available for clinical visits on request. Bundled Living Care fees themselves are generally not submittable to insurance.

4.2 Indemnification

You agree to indemnify and hold harmless Hillary Lin, MD and her affiliates, agents, and representatives from any claims, losses, liabilities, damages, costs, or expenses (including reasonable attorneys' fees) arising from: (1) your use of the Website or services; (2) your violation of these Terms; or (3) your misrepresentation of your physical location or eligibility for services.

4.3 Dispute Resolution

We encourage you to contact us first with any concerns at hello@hillarylinmd.com. For disputes that cannot be resolved informally, the parties agree to attempt mediation before pursuing other legal remedies.

For disputes arising from health coaching services, any unresolved claim shall be submitted to binding arbitration under the rules of the American Arbitration Association, except that either party may seek emergency injunctive relief in a court of competent jurisdiction. All claims must be brought in your individual capacity, not as a class member.

For disputes arising from medical services, dispute resolution is governed by the applicable laws and medical practice statutes of the state in which services were rendered. Nothing in this section limits your rights under state medical liability law.

4.4 Governing Law

These Terms are governed by and construed in accordance with the laws of the State of New York, without regard to conflict of law principles, except where superseded by the mandatory law of the state in which medical services were rendered.

4.5 Changes to These Terms

We reserve the right to modify these Terms at any time. Material changes will be communicated via the Website or email. Continued use of the Website may constitute acceptance of updated Website-use terms, but it does not silently amend a version-bound commercial authorization or the accepted pricing and service contract for a current paid term. Material changes to a future purchase, renewal, or migration require affirmative acceptance when applicable. The "Last Updated" date at the top of this page reflects the most recent revision.

4.6 Severability

If any provision of these Terms is found to be invalid or unenforceable, that provision will be modified to the minimum extent necessary to make it enforceable, and the remaining provisions will continue in full force and effect.
