There are over 300 names for sugar. Dr. Lin opens with her own recalibration on that number — she thought it was 50, then 100, then checked again — because it is the cleanest illustration of what this episode is about. Nutrition labels are not simply hard to read; several of their features are actively engineered around, and knowing which parts are regulated and which are marketing is most of the skill.
The mechanics are specific. Ingredients are listed in descending order by weight, so splitting sweetness across corn syrup solids, fructose and molasses keeps every one of them out of the top few slots while the total stays high. Serving sizes are set by the manufacturer, so a bottle of iced tea becomes two and a half servings and a value bag of trail mix becomes 28. Trans fat can be rounded to zero below a threshold, so a product can advertise zero grams and still deliver a meaningful dose across a realistic portion.
The terms carry different weights too. Natural, lightly sweetened, multigrain and made with real fruit have no enforceable definition. No cholesterol on a peanut butter is true and meaningless, since cholesterol only occurs in animal products. Dr. Lin also spends real time on fiber, where the distinction between the viscous fibers with outcome data and the non-viscous ones manufacturers add to hit a number is the difference between a benefit and a marketing claim — plus sodium, MSG, rBST and a closing set of shopping tactics, including her own failed attempt to hit the saturated fat target by diet alone.

- The front of the package is where the unregulated language lives, and most of it means less than it appears to. Natural has no enforceable definition, so a product carrying it can still contain preservatives and additives. No added sugar routinely means fruit concentrate instead, which still moves glucose and insulin. Made with whole grains often means a small amount of whole grain mixed into mostly refined — the phrase to look for is 100% whole grain, and Dr. Lin disregards multigrain entirely, since multiple refined grains still qualify. Lightly sweetened has no regulatory meaning at all.
- Two label claims are technically accurate and still pointless. No cholesterol on a plant product is a statement about a molecule that only occurs in animal foods — cholesterol-free peanut butter was never going to contain any. Gluten-free on rice is the same move. Dr. Lin's position on both is that they trade on outdated dietary beliefs; on cholesterol specifically, dietary intake is broken down and recycled rather than converted directly, and the more useful thing to watch if you are at cardiovascular risk is saturated fat. Gluten genuinely matters for celiac disease and true intolerance, and for everyone else she considers refined carbohydrate the larger problem.
- Serving size is the first line on the panel and the one that invalidates everything below it if you skip it. A cereal listing 8 g of sugar per serving is describing a quarter cup, and most people pour a full cup. A bottle of iced tea is commonly two and a half servings, consumed in one go. Her worked example is a value-size bag of trail mix at 140 calories per one-ounce serving and 28 servings in the bag — 3,920 calories, roughly two days of intake, in a package one person can finish on a hike. She is candid that doing this arithmetic is tedious, and recommends at minimum eyeballing the serving count before reading anything else.
- The ingredient list is ordered by weight, which is precisely why sugar gets fragmented. Manufacturers keep any single sweetener out of the first few positions by using several in smaller amounts — corn syrup solids, crystalline fructose, molasses, brown rice syrup, evaporated cane juice, agave nectar, barley malt, fruit juice concentrate — so sugar appears at positions five, six and seven while the cumulative load is unchanged. Dr. Lin's image is a party guest who keeps swapping name tags. The practical consequence is that scanning for the word sugar near the top is not sufficient.
- The added sugar hides most successfully in products marketed as healthy. Flavored yogurts can exceed 20 g per serving, about five teaspoons, and some carry more sugar than a can of soda while selling on protein content. Whole-grain cereals and granola bars frequently carry multiple sweetener forms. The American Heart Association's ceiling is 25 g of added sugar daily for women and 36 g for men, and Dr. Lin's own position is that the target should be as close to zero as you can manage, since carbohydrate intake already supplies plenty. Her practical route is unsweetened versions plus sweetening yourself if you want it — she orders boba at 0% sugar with no pearls — and she notes taste buds turn over roughly every two weeks, so the adjustment period is short.
- The fiber section is the part most people have never encountered, and it changes which products are worth buying. Manufacturers add synthetic fibers to reach a high fiber claim, but non-viscous soluble fibers such as inulin, fructooligosaccharides and wheat dextrin do not deliver what the viscous fibers do. High-viscosity fibers — psyllium and beta-glucan — form a gel in water that slows digestion, and these are the ones with evidence for lowering cholesterol and improving glycemic control. The non-viscous versions also fail to add stool water content, so they do not help constipation, and fermentable fibers like inulin and oligofructose may not affect satiety while causing bloating and gas. She notes the American Gastroenterological Association and American College of Gastroenterology have observed that finely ground wheat bran can actually harden stool relative to coarser fiber.
- The zero trans fat claim has a rounding loophole and a manufacturing loophole. Below a small threshold per serving, trans fat can be declared as zero — so a product eaten in multiple servings can deliver a real dose from a label reading nothing. Separately, even where partial hydrogenation is banned, high-temperature processing can generate trans fats after the fact, meaning they were never an ingredient to declare. Partially hydrogenated is still the term to scan for, particularly on products bought while travelling, and the usual locations are baked goods, margarine, microwave popcorn and fried fast food.
- On saturated fat Dr. Lin discloses her own failure, which is the most useful thing in the section. She has genetically elevated LDL-C and tried to control it by diet alone, targeting the AHA's roughly 3 g of saturated fat per day. She found it close to impossible — a few eggs puts you over before dairy or anything else — and now uses medication she is happy with. Her simplification is to cook everything in olive oil and stop doing the arithmetic, on the grounds that replacing saturated with polyunsaturated fat has solid evidence behind it and the mental overhead of tracking grams is not worth it. Palm and coconut oil are the plant sources she flags alongside fatty meat and full-fat dairy.
- Sodium is risk-stratified rather than universal, and it hides in foods marketed as preservative-free. A single can of soup can approach 1,000 mg, around two thirds of the stricter daily target on its own, and canned vegetables often carry added sodium precisely as a preservative. The AHA limit is under 1,500 mg daily for people with high blood pressure, chronic kidney disease, diabetes, or elevated background risk, and Dr. Lin notes higher hypertension and heart disease risk in African-American populations. For healthy adults the target is under about 2,300 mg. Her framing of MSG is a shortcut rather than a scare: treat it as very concentrated sodium, which puts it in a similar risk category rather than the toxin category the media assigned it — while noting animal work associating it with hypothalamic damage affecting hunger regulation, leptin resistance, and raised blood pressure and renal dysfunction in hypertensive individuals.
- Emulsifiers and preservatives are worth learning by name, because they appear in exactly the products people switch to for health reasons. The gums, carrageenan, polysorbate 20 and 80, and carboxymethyl cellulose are what make plant milks, ice cream, creamy dressings and processed desserts feel good in the mouth — and Dr. Lin's frustration is personal, since the smoother nut milks she went looking for turned out to be smooth because of what was added. The concern is gut barrier function and the resulting inflammation, with observational work linking certain emulsifiers to cardiovascular disease and to overall cancer risk. On preservatives, BHT and BHA in cereals, chips and processed meats are the common ones, with natural alternatives like vitamin E and rosemary extract as what to look for instead. Her fair caveat is that regulators set limits per substance, and the concern is cumulative exposure across a diet built on ultra-processed food.
- The shopping tactics are where the episode lands, and several cut against common assumptions. Organic is not a health claim — organic cookies are still cookies, and the label says nothing about sugar or fat. The frozen aisle is genuinely good for produce, since fruit and vegetables are frequently picked and frozen at peak ripeness with no preservatives required. Hydroponically grown produce is grown in controlled environments without pesticides, which sidesteps the organic question entirely, though Dr. Lin is honest about the price. Beyond that: shop the perimeter, bring a list, treat low-fat and light as signals that sugar replaced the fat, and batch cook so you control the oil and salt yourself. Her simplest heuristic is that food with no nutrition label at all is usually the best sign.

No, and the difference is measurable rather than philosophical. Dr. Lin notes that medical bodies internationally recommend avoiding trans fats entirely, since there is no argument for any intake.
How much sodium is too much?
It depends on who you are, which is not how most sodium advice is framed. The American Heart Association's stricter target of under 1,500 mg a day applies to people with high blood pressure, chronic kidney disease or heart disease, people at elevated risk, and older adults — Dr. Lin also notes higher hypertension and cardiovascular risk in African-American populations. For healthy adults with none of those factors, the general target is under roughly 2,300 mg. The reason this is harder than it sounds is that the sodium sits in packaged and restaurant food rather than the salt shaker: a single can of soup can approach 1,000 mg, and canned vegetables often carry sodium specifically as a preservative, including in products marketed as preservative-free.
Should I avoid milk from cows treated with rBST?
Dr. Lin's read is that the strongest concerns are about the cows and about antibiotic resistance, with the direct human evidence weaker than the marketing suggests. Recombinant bovine somatotropin raises milk yield but increases mastitis risk in treated cows, which drives antibiotic use — and antibiotic overuse in livestock contributes to antimicrobial resistance, a serious public health problem given how slow new antibiotic development is. Treated cows also show altered metabolic parameters and reduced pregnancy rates. On the human side, studies indicate rBST does not meaningfully change milk composition or leave antibiotic residues, and the residual concern is elevated IGF-1 in the milk — though she notes the biological activity of orally consumed IGF-1 is minimal. Those concerns were enough for the EU and Canada to ban it; in the US, labelling makes it avoidable, and organic certification prohibits synthetic hormones outright.
