Eye bags and dark circles get treated as one problem with one cause, and they are neither. Getting this wrong means applying a melanin-targeting product to a circulation problem.
- Sleep deprivation is a real contributor but a poor explanation. Dr. Lin also notes variation in how the whole periorbital region ages: Asian populations tend toward earlier eye bags given how orbital fat and skin laxity develop, Mexican women toward more pronounced crow's feet and lower eyelid wrinkles, and Han Chinese women toward muscle thinning and fat shifting around the eye.
- Several common medications cause dark circles, and most people are never told. Prostaglandin analogues used in glaucoma treatment β latanoprost (Xalatan), travoprost (Travatan) and bimatoprost (Lumigan) β increase melanin content in periorbital melanocytes, and the effect is usually reversible on discontinuation. Dr. Lin points out the same drug class is used for eyelash growth, where the pigmentation side effect is rarely mentioned before people start. Antimalarials and heavy metal contamination can also produce drug-induced pigmentation.
- Skin-lightening creams can produce the opposite of their intended effect. Prolonged use of hydroquinone can cause exogenous ochronosis, a persistent blue-black pigmentation, and it is particularly prevalent in people with darker skin tones using these products over extended periods. Dr. Lin's framing is that the skin, repeatedly lightened, rebels β and her practical instruction is to use these agents under medical supervision rather than indefinitely on your own.
- The health signals worth acting on are specific and testable. Dr. Lin cites studies finding that people with dark circles had significantly lower vitamin D levels and higher insulin resistance than others, and that B12 deficiency can increase melanin production through oxidative stress. Iron deficiency belongs on the same list. Insulin resistance drives oxidative stress and inflammation, both of which stimulate melanin β meaning the circles may be reporting a metabolic problem rather than a cosmetic one.
- Persistent dark circles despite eight hours in bed should prompt a question about sleep quality rather than quantity. Sleep apnoea lowers oxygen levels intermittently through the night, which dulls the skin and makes periorbital vessels more visible. Dr. Lin's specific advice is that if you consistently wake unrested with dark circles, a sleep study is a more useful next step than another eye cream.
- Allergic shiners are worth treating properly rather than concealing. Dr. Lin's escalation path goes beyond antihistamines and allergen avoidance to intranasal steroids, and to immunotherapy for people who want to address the allergy itself rather than manage it seasonally.
- Puffiness is often fluid rather than fat, and the causes are worth separating. High sodium intake, alcohol, hormonal changes and sleeping position all drive periorbital fluid accumulation β sleeping on your stomach or side pools fluid in the lower eyelids overnight. More seriously, kidney or liver dysfunction impairs fluid handling and can show up here first. Hypothyroidism produces both periorbital puffiness and skin thinning, and chronic cortisol elevation drives fluid retention while breaking down collagen.

Dr. Lin's sequencing is deliberate: sleep, hydration and nutrient correction, allergy management, stress and hormone assessment, and antioxidant support come first, because addressing those improves overall health rather than just appearance. If bags persist after that, the surgical options include blepharoplasty to remove or reposition fat and excess skin, sometimes combined with autologous fat grafting for volume loss, and tear trough ligament reset β which she notes has shown superior results for tear trough deformity and dark circles compared with traditional eye bag surgery.
